Final Forms is the online platform for all North Ridgeville student athletes grades 7 through 12. You will NOT need to use Privit for athletics. Final Forms will replace that program.
Families and student athletes will need to register on Final Forms.

Pages 3 and 4 of the Physical form will be turned into the Athletic office or Trainer.
